To become a member of the Connecticut Technology Council, you must complete the following form. You will need to classify your company according to the four membership types below. A technology company is a Corporate Member, a professional services firm is an Associate Member, a college or university is an Academic Member and an non-profit is eligible to join as an Affiliate Member.
New members can pay thier membership by credit card or send the Technology Council a check based on the dues calculated within your membership category. If you have any questions you can contact Mike Scricca, Membership Director.

The parent company info should be altogether.
Headquartered in CT y/n question, IF NO, it should be followed by Name and city/state of parent company
Also, the questions just for Software/IT and Advanced manufacturing are questions that should only pop up if they are S/IT or AM based on their selection of industry type. IF SOFTWARE / IT, then ask the s/IT questions.
I realize this may be hard programming - to make the questions pop-up only IF... We can have them show for all if it is significantly easier for programming.
